Ozark Valleys Community Services, as the provider of transportation agrees to the following practices:

 

Ozark Valleys Community Services Posts Title VI notices on our agency website, in public areas of our agency, in our conference room, and on our buses and/or paratransit vehicles.

 

Ozark Valleys Community Services operates its programs and services without regard to race, color, or national origin, in accordance with Title VI of the Civil Rights Act of 1964.

If you believe you have been discriminated against on the basis of race, color, or national origin by Ozark Valleys Community Services, You may file a Title VI complaint by completing, signing, and submitting the agency’s Title VI complaint form.

How to file a complaint with Ozark Valleys Community Services:

  1. To obtain a form you may call 573-546-2418 or go to the agency website at http://www.ovcsmo.org

  2. In addition to the complaint process at Ozark Valleys Community Services, complaints may be filed directly with the Federal Transit Administration, Office of Civil Right, 1200 New Jersey Avenue SE. Washington D.C. 20590.

  3. Complaints must be filed within 180 days following the date of the alleged discriminatory occurrence and should contain as much detailed information about the alleged discrimination as possible.

  4. The form must be signed and dated, as include your contact information.

I have read the Title VI Antidiscrimination statement above and agree to refrain from discrimination as outlined. I understand the full Antidiscrimination plan is on file and is available for my reading if I so choose. I also understand that as an employee of OVCS which provides transportation, if I become aware of any violation of this information it is my responsibility to report it immediately to the Director and that failure to do so will result in progressive discipline up to and including termination.
